Assertive Community Treatment

Assertive Community Treatment is designed to provide treatment, rehabilitation, and support to individuals who are diagnosed with a severe mental illness and whose needs have not been well met by more traditional mental health services. The ACT team includes members from the fields of social work, nursing, psychology, substance abuse, and psychiatry. Services such as individual counseling, group counseling, personal support, case management, medication delivery, and medication management are provided in the community, rather than an office or hospital setting. Admission criteria includes: 18 years of age or older; 3 or more hospitalizations in a psychiatric unit/hospital in the past 12 months, OR 5 or more in the past 24 months, OR 180 days of total length stay in a psychiatric unit/hospital within the past 12 months, and have an eligible diagnosis as determined by APS and be in eligible disability group of Mental Health OR Mental Health and Substance Addiction. Goals of ACT include: helping individuals with serious mental illness understand their illness and control symptoms; assisting individuals with co-occurring substance abuse and serious mental illness understand their illness, control their symptoms, and avoid substances; promoting independence and encourage individuals to be active in their community; preventing and/or decreasing psychiatric hospitalizations.

Community Engagement

This Program provides the necessary supports needed to maintain an individual in his or her community and to prevent hospitalization. CES assist consumers with activities of daily living such as grocery shopping, transportation to doctor and counseling appointments, coordination of services to community agencies and resources, assistance with medication compliance, etc. Any consumer in crisis qualifies automatically for this service. We also have a CES that serves as a Hospital Liaison that works with consumers that have been hospitalized through commitment or crisis to help with discharge planning and continuum of services.
