This program is a Medicaid service that is aimed at persons with a bipolar diagnosis which places them at increased risk for hepatitis. The team consists of a social worker, a registered nurse, a behavior specialist, and a psychiatrist. Clients who are enrolled in this program are contacted monthly to assess progress. The clients are given resources to access for needs such as primary care, transportation, assistance services, and tobacco cessation.

Southern Bluestone Integrated Health Services

Southern Highlands CMHC and Bluestone Health Center joined forces to create Southern Bluestone Health Services, a primary health clinic which provides services to consumers of Southern Highlands. The program is grant funded and the goal is to integrate psychiatric and primary health services as well as improve the overall wellness of clients. The core team consists of a nurse practitioner, a registered nurse, a peer support coach, and a wellness coordinator. Wellness activities are offered to participants along with incentives for compliance. The four areas of wellness that are highlighted are : Nutrition/Exercise, Smoking Cessation, Chronic Disease Self Management, and Heart Health. To be eligible for this program a client must be an active patient at Southern Highlands CMHC and be willing to complete a brief interview process, and have fasting labs drawn yearly.
