Integrated Health

Integrated health care at our CCBHC ensures that physical and behavioral health needs are addressed together for whole-person wellness. We provide on-site primary care screenings and connect individuals with ongoing primary care providers to promote early intervention and continuity of care.

At our Princeton Clinic, primary care services are located within the 200 12th St Extension building, making access seamless and supporting coordinated, collaborative treatment. We make primary care referrals a priority!

Why Integrated Care Matters

Mental and physical health are deeply connected. Depression can worsen diabetes, anxiety can raise blood pressure, and chronic pain can lead to mental health challenges. Our integrated approach recognizes these connections and treats the whole person, not just individual symptoms.

By bringing mental health and primary care together, we ensure that nothing falls through the cracks and you receive comprehensive, coordinated care that addresses all your health needs.
